Overview This short webinar will be delivered by the Clinical Mentor from The Advance ProjectTM team. The Advance Project TM resources encourage team work within general practice to implement Advance Care Planning and assessment of palliative care/ supportive care needs for your patients.

This CPD activity is an introduction of The Advance ProjectTM , its further online modules and resources. It identifies the importance of Primary Settings in the early access of Advance Care Planning and Palliative and Supportive Care.

Learning areas

  • Utilise the ‘Advance Project Guide’ and resources to identify patients suitable for an introduction to advance care planning (ACP) and/or an assessment of their palliative/supportive care needs  
  • Utilise the Advance Project evidence-based resources to introduce ACP and assess patients’ and their carers’ palliative and supportive care needs     
  • Evaluate patient’s needs for follow up regarding ACP and/or any additional support required to ensure their palliative/supportive care needs are met
